Rebalance MLD and Massage requires a benefit check and short interview, either over the phone or by email, before scheduling or full payment will be expected at time of service if checklist below is not complete. After the benefit check is complete, be ready to submit a letter of medical necessity or referral from a diagnosing provider. Insurance card and a valid form of legal ID must be presented at the first appointment.

At this time Rebalance MLD and Massage offers direct billing for the following insurance companies

Anthem, Blue Cross Blue Shield, Regence*, and United Health Care

*In-network with Anthem/BCBS/Regence of Oregon, Washington, and California only. For many plans administered outside of those states it may be covered under out-of-network benefits or may not be covered depending on plan.

We can provide a superbill for all other medical massage services which can be submitted for reimbursement for those who have out-of-network coverage and for your records. Billing codes are only provided for medical massage services.

Insurance plans which do have massage (CPT code 97124) or manual therapy (CPT code 97140) benefits typically only cover massage and manual therapy services which are deemed medically necessary. Rarely are relaxation/stress relief sessions, also known as wellness massage, covered. Determination of medical necessity must come from a diagnosing provider such as a primary care physician, nurse practitioner, doctor of a specialty, or other medical professional. All insurance companies in which Interconnect Bodyworks LLC dba Rebalance MLD and Massage require massage therapists/massage therapy practices to have a letter of medical necessity or referral on file to cover massage or manual therapy sessions. Please be ready to provide one.
